Overview

Quality Health Outcomes, LLC (“QHO”) is an independent, non-clinical healthcare navigation and medical oversight firm. QHO does not provide medical advice, diagnosis, or treatment, and does not bill insurance. QHO is not a healthcare provider, health plan, or healthcare clearinghouse, and is therefore not a “covered entity” under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

While not a HIPAA-covered entity, QHO voluntarily maintains HIPAA-aligned privacy, security, and confidentiality practices to protect sensitive personal and health-related information entrusted to us.

Information We Collect

QHO may collect personal, administrative, and health-related information voluntarily provided by clients or their authorized representatives for the limited purpose of healthcare navigation, coordination, record organization, second-opinion facilitation, and advisory support.

Clients are solely responsible for ensuring that they have the legal authority to share any information provided to QHO.

Permitted Use of Information

Information collected by QHO is used strictly to:

  • Deliver requested non-clinical services

  • Organize and summarize medical records

  • Coordinate communications with third parties at the client’s direction

  • Support informed decision-making

QHO does not make clinical decisions, direct care, or substitute for licensed healthcare providers. All diagnosis, treatment, and medical decision-making remain exclusively between the client and their licensed clinicians.

Information Sharing & Disclosure

QHO does not sell, rent, trade, or monetize personal or health-related information. QHO does not accept referral fees and maintains no financial relationships with hospitals, physicians, pharmaceutical companies, or medical device manufacturers.

Information is disclosed only:

  • With explicit client authorization

  • To third parties designated by the client

  • As required by law, court order, or legal process

Data Security Safeguards

QHO implements administrative, technical, and physical safeguards designed to protect confidentiality, integrity, and availability of information, including but not limited to:

  • Access controls

  • Secure storage and encrypted transmission where feasible

  • Confidentiality obligations for contractors and partners

  • Policies designed to minimize unauthorized access or disclosure

No system is completely secure. By engaging QHO, clients acknowledge and accept the inherent risks associated with electronic communications and data storage.

Retention & Destruction

Information is retained only as long as reasonably necessary to fulfill service obligations or comply with legal requirements, after which it is securely deleted or destroyed in accordance with internal data handling policies.

Confidential Communications

All communications may contain confidential, proprietary, or privileged information intended solely for the named recipient. Unauthorized review, use, disclosure, copying, or distribution is strictly prohibited. If received in error, please notify the sender immediately and permanently delete all copies.

Limitation of Liability

To the fullest extent permitted by law, QHO disclaims liability for unauthorized access, disclosure, or loss of information beyond its reasonable control, including risks inherent to electronic transmission or third-party systems used at the client’s request.

Policy Updates

QHO reserves the right to modify this Privacy Policy at any time. Continued use of services constitutes acceptance of the current policy.
